Golden Days Primary Care Program is our flagship health care program catered to the preventive care the individuals and families that join our practice. Our Primary Care providers provide both the first contact for a person with an undiagnosed health concern as well as continuing care of varied medical conditions, not limited by cause, organ system, or diagnosis.

Golden Days Healthcare prides itself on providing full preventive care primary care services to individuals and families in the community and surrounding areas. We ensure to diagnose, treat and consistently grow with you and your healthcare needs as they arise.

  • Patient focus—First and foremost, you as the patient are central to primary care. Primary care as a practice gives you complete, coordinated care and support to help you live healthfully.
  • First contact—Your primary care provider is your first point of contact when you need care. This provider can be a medical doctor (M.D.), nurse practitioner (N.P.), or physician assistant (P.A.).
  • Group effort—While you may have one primary care provider, it's not uncommon for your provider to be part of a practice that includes physicians and non-physician health professionals who can step in if your doctor isn't available and you need help. These folks are your healthcare team and work together to give you quality care.
  • Patient empowerment—Your provider is your partner, which means you also play a significant role in your own care. Through the education and information your provider shares with you and your own research, you'll be empowered to take charge of your health in a proactive, prevention-focused way.
  • Health promotion—Primary care isn't limited to just getting care when you're sick (though that's part of it). It also includes things like health maintenance, counseling, and education to help you stay healthy.
  • Disease prevention—Your primary care provider works with you to help prevent big health problems. Through regular check-ups and open conversation about your health and lifestyle habits, your provider can help you prevent chronic conditions such as diabetes, heart disease, and cancer.
  • Care coordination - You may experience health conditions that require more care, such as specialty care, chronic disease management, or hospitalization. Your primary care team helps coordinate your care and makes sure important information is shared with all those who are caring for your health.
  • Healthcare advocacy—Your primary care provider can help you navigate the healthcare system and figure out how to meet your healthcare needs in a cost-effective way. This includes working with other providers involved in your care, as well as with your health insurance company.

For sick or urgent visits, appointments are available the same day or within 24 hours. New patient physicals we suggest at least 1 day in advance. We would like to ensure you are fasting properly for lab work and the provider has a time reserved for you to complete an extensive visit and address all medical concerns.

Please plan to arrive 15 minutes prior to your scheduled appointment time if you have yet to complete new patient form(s). You will be greeted by our front desk attendant as you “check-in” at the front desk. You will be asked to provide your insurance card and a photo ID. Shortly thereafter you will be called back to the exam room by a medical assistant or medical provider who will take your vitals and make sure you are ready for the provider to see you.

You will be responsible for any co-pays reflected on your insurance plan card. If you have specific questions concerning your benefits, please call the member services phone number on your insurance card.

Our “check-out” person will schedule any follow-up appointments as directed by your provider, and provide you with any prescriptions or referrals you may need.
